Terms of Service

These Terms of Service (“Terms”) govern your access to and use of the services provided by Lumen Private Services (“Lumen,” “we,” “us,” or “our”). By submitting a consultation request, engaging our services, or otherwise interacting with Lumen, you agree to be bound by these Terms.

If you do not agree to these Terms, please do not use our services.

1. Scope of Services

Lumen Private Services provides non-clinical, administrative healthcare coordination services. Services may include, but are not limited to:

  • Scheduling appointments and coordinating with providers

  • Tracking referrals, labs, records, and follow-ups

  • Administrative communication with provider offices and insurers

  • Assistance with forms, billing inquiries, and logistics

  • Written summaries of coordination activities

Lumen does not provide medical advice, clinical opinions, diagnoses, treatment recommendations, or emergency services.

2. No Medical Advice or Provider Relationship

Lumen is not a healthcare provider, medical practice, or insurance provider. Engagement with Lumen does not create a doctor–patient or clinician–patient relationship.

All medical decisions remain solely between you and your licensed healthcare providers. Any information shared by Lumen is administrative in nature and should not be relied upon for medical decision-making.

3. Emergency & Time-Sensitive Matters

Lumen does not provide emergency or urgent care coordination. Our services are not monitored continuously.

If you are experiencing a medical emergency or urgent health concern, call 911 or your local emergency services immediately.

4. Client Responsibilities

To ensure effective coordination, clients agree to:

  • Provide accurate, complete, and timely information

  • Maintain active relationships with their healthcare providers

  • Respond within reasonable timeframes when input or authorization is required

  • Understand that delays caused by third parties (providers, insurers, facilities) are outside Lumen’s control

Lumen is not responsible for outcomes resulting from incomplete, inaccurate, or delayed client-provided information.

5. Authorization to Communicate

By engaging Lumen, you authorize us to communicate with healthcare providers, facilities, and insurers for administrative coordination purposes only, as permitted by you and applicable law.

You are responsible for executing any required HIPAA authorizations or consent forms requested by providers or insurers.

6. Privacy & Confidentiality

Lumen respects client privacy and maintains reasonable administrative safeguards for information shared with us.

Please refer to our Privacy Policy for details regarding information collection, use, and storage. While Lumen takes confidentiality seriously, no electronic communication or storage system can be guaranteed to be 100% secure.

7. Fees & Engagement Structure

Services are provided on a private, retainer-based engagement, unless otherwise agreed in writing.

  • Fees are disclosed prior to engagement

  • Payment terms are outlined in the engagement agreement

  • Fees are non-refundable once services are rendered

Lumen reserves the right to pause or terminate services for non-payment.

8. Availability & Communication Boundaries

Lumen provides services during defined business hours, which will be communicated at engagement.

We do not guarantee immediate responses outside of agreed-upon availability windows. Boundaries are essential to providing consistent, high-quality coordination.

9. Limitation of Liability

To the fullest extent permitted by law, Lumen shall not be liable for:

  • Medical outcomes or provider decisions

  • Acts, omissions, delays, or errors of third parties

  • Indirect, incidental, consequential, or special damages

Lumen’s total liability for any claim related to services shall not exceed the fees paid to Lumen for the applicable service period.

10. Termination

Either party may terminate the engagement pursuant to the terms outlined in the engagement agreement.

Upon termination, outstanding fees remain due, and Lumen will provide a reasonable transition of administrative information upon request.

11. Governing Law

These Terms are governed by and construed in accordance with the laws of the state in which Lumen Private Services operates, without regard to conflict-of-law principles.

12. Changes to These Terms

Lumen may update these Terms from time to time. The most current version will be posted on our website. Continued use of services after changes constitutes acceptance of the revised Terms.
